Community Matron

The Community Matron is an experienced senior nurse who provides nursing care and supports patients who have one or more long term condition and complex care needs.

The Community Matron acts as a ‘case manager’ for patients, co-ordinating care and working collaboratively with other members of the multi-disciplinary team to provide a whole systems approach to managing health care.

The Community Matron monitors and works with patients to try and identify and treat early any deterioration in their condition, thereby preventing unplanned hospital admissions.
