Air Balloon Surgery can take patients who live within the surgery
catchment area including:
The clinical staff work together as a team to treat illnesses, to
provide help with health related problems, and to offer advice on staying
healthy. However, if you have a preference for a particular GP the
practice will try to meet your choice although this cannot be guaranteed.
Please inform the receptionist of your preference when you register.

This form should take 10
minutes to complete and it will take about 3 working days to get you
registered. If at any point you find this form too hard, you can visit the
practice to register a different way.
Alternatively, you can
attend the surgery and ask reception for a new registration form. However, it may take longer to process your
application to register.
During registration you will be
asked to provide us with useful information about your medical background and
current health.
If you have an on-going problem
e.g. diabetes, high blood pressure, asthma, etc, our Care Navigators will be
able to explain monitoring procedures and refer you to the appropriate
clinicians.
